29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface were not used:    a) Job site, 2278 Miles Avenue, Pittsburgh, PA: On or about August 1, 2012, during roofing work on a home, the employer did not ensure that a safe means of accessing the ladder jack scaffold was provided in that the picks were set up on the outside of the ladder frame, forcing workers to climb over them and in the process, exposing them to fall hazards of approximately 20 feet.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:       a) Job site, 2278 Miles Avenue, Pittsburgh, PA: On or about August 1, 2012, during roofing work on a home, the employer did not ensure that fall protection systems/devices were used while working on a ladder jack scaffold where exposed to fall hazards of approximately 20 feet.
