29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:    At the project on or about August 1, 2012, and at time prior thereto, appropriate hand protection was not used when mixing and applying QuickCrete Mortar that contained Portland cement.  Gloves were used with the palm sides made of rubber instead of nitrile or butyl.  In addition, some employees were found not to be wearing eye protection.

29 CFR 1910.1200(f)(5): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged, or marked with the identity and appropriate hazard warnings regarding the chemicals contained therein: (Construction Reference: 1926.59):    At the project on or about August 1, 2012, and at times prior thereto, the employer allowed workers to use a portable unmarked soap dispenser to store and access pipe threading oil.

29 CFR 1910.1200(h): Employees were not provided effective information and training as specified in 29 CFR 1910.1200(h)(1) and on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard was introduced.       At the project, Anderson Air Force Base, Yigo, Guam - On or about August 1, 2012, and at times prior thereto, employees were not trained on the hazards of products containing Portland cement.  Employees were not trained on personal protective equipment, and health and safety hazards associated with these products.
