29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:    a. On or about August 10, 2012, employees working from the basket of an aerial lift were exposed to a 17 feet fall hazard, in that, fall protection such as a belt and lanyard was not provided and no other means of fall prevention were utilized.
