29 CFR 1910.147(c)(1): The employer did not establish a program consisting of energy control procedures, employee training, and periodic inspections to ensure that before any employee performed servicing or maintenance on a machine or equipment, would be exposed to unexpected energy, startup, or release of stored energy which could cause injury. The machine or equipment shall be isolated from the energy source and rendered inoperative.     Safety and health program: The employer had not established or implemented a Lockout/Tagout program with machine specific procedures, training and annual inspections of the program to ensure that it was appropriate for protecting employees who perform service and/or maintenance on machinery or equipment. Maintenance employees perform work on machines daily while servicing equipment, such as but not limited to, air separators, conveyor belts, balers, etc.  The energy sources associated with the equipment includes pneumatics, electricity, and hydraulic power.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met.    Written hazard communication program: The employer exposed employees to chemical hazards due to the company not developing or implementing a written Hazardous Communication Program prior to using chemicals, such as but not limited to, propane, diesel, propane, engine oil, and grease.  Employees are required to work with chemicals within the establishment to maintain production and operations.
