29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift. On August 15, 2012 and times prior to, an employee was working in an aerial lift without using a belt or lanyard. This condition exposed the employee to a fall hazards up to 10 feet to the ground below.
