29 CFR 1926.307(d)(1): Pulley(s) with part(s) seven feet (2.128 m) or less from the floor or work platform were not guarded in accordance with the requirements specified in paragraphs (m) and (o) of this section: At the site, dewatering pump drive unit, make and model unknown, on August 13, 2012 pulleys were unguarded exposing employees to caught by and amputation hazards during operation and fueling of the machine.
