29 CFR 1926.405(a)(2)(ii)(E): Lamps used in temporary wiring for general illumination were not protected from accidental contact or breakage: (a) James River Interiors, Inc., Fort AP Hill Army Reserve Training Center project, men's locker room. On or about August 16, 2012, temporary lighting used in the area where drywall was being installed did not have the incandescent bulbs protected from accidental breakage in that the protective guards were missing.

29 CFR 1926.405(a)(2)(ii)(F): Temporary lights were suspended by their electric cords when such lights and cords were not designed for suspension: (a) James River Interiors, Inc., Fort AP Hill Army Reserve Training Center project, men's locker room. On or about August 16, 2012, temporary lighting used in the area where drywall was being installed was suspended from the ceiling by wrapping metal wire around the flexible cord, and not at the designated suspension point at the lamp holder.
