29 CFR 1926.453(b)(2)(v): A body belt was not worn with a lanyard attached to the boom or basket while working from an aerial lift: a) Horizon Renovations & Millwork, Inc. on or about August 16, 2012, at the address of 630 S. Reynolds Rd. Toledo, OH: The employer failed to ensure employees working from an aerial lift were adequately protected from falls. Employees working from the lifts installing tile work were not clipped off to the basket while the platform was elevated. The employees were exposed to an approximately 24 ft. fall to the ground below.
