29 CFR 1910.23(d)(1)(ii):     Stairway(s) less than 44 inches wide having one side open, did not have at least one stair railing on the open side:      (a)  Upper Maintenance Area:  A wooden stairway leading to the roof was not equipped with a standard railing in that it was not equipped with an intermediate railing.  The stairway measured 26 and 1/4 inches in width and was equipped with 14 risers, as observed on August 14, 2012.

29 CFR 1910.212(a)(1):     One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    (a)  Lower New Maintenance Area:  Employees operated a Craftsmen 17 inch, 16 speed drill press that was not equipped with machine guarding over the rotating chuck, as determined on August 14, 2012.

29 CFR 1904.29(b)(1):    A Log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent), was not completed in the detail as required by the regulation:    (a)2011 OSHA Form 300:  The employer did not record an occupational hearing loss for an employee who had a standard threshold shift in the left ear and had a total hearing level loss of 30.3 decibels (dB) (with age correction) above audiometric zero (averaged at 2000, 3000, and 4000 Hz) in the same ear, as required per 29 CFR 1904.10(a), as determined on August 14, 2012.      Abatement Certification Required for this Item.
