On or about August 20, 2012: at 1330 Sunnys Halo Drive, Suwanee, Georgia:    An employee framing a house was exposed to being struck by tools and materials that could fall from the employee working above him when the employee did not use a hard hat.

On or about August 20, 2012, at 1330 Sunnys Halo Drive, Suwanee, Georgia:    Three employees framing a house using nail guns, and saws were exposed to struck-by hazards from tools and materials while working without eye protection.

29 CFR 1926.501(b)(13):     Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b)  On or about August 20, 2012, at 1330 Sunnys Halo Drive, Suwanee, Georgia:    a) An employee decking a porch was exposed to a fall hazard that ranged from approximately 10.5-feet at the front of the porch to 12.8-feet at the rear of the porch. The employee did not use fall protection.     b) The above mentioned employee and the owner/crewleader working on the second floor of the house were exposed to a fall hazard from an approximately 7-foot by 13.4-foot unguarded floor opening that ranged from 9-feet to 10.4-feet.

29 CFR 1926.503(a)(2):     The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii)    On or about August 20, 2012, at 1330 Sunnys Halo Drive, Suwanee, Georgia:    a) An employee decking the porch of a house was exposed to a fall hazard that ranged from approximately 10.5-feet from the front of the porch to approximately 12.8-feet at the rear of the porch. The employer did not train the employee to recognize and minimize fall hazards.     b) The above mentioned employee and the owner/crew leader were exposed to an unprotected floor opening that was approximately 7-feet by 13.4-feet and ranged from approximately 9-feet to 10.4-feet while working on the second floor.  The employee was not trained to recognize and minimize fall hazards.

On or about August 20, 2012, at 1330 Sunnys Halo Drive, Suwanee, Georgia:    An employee framing a house was exposed to a fall hazard that ranged from approximately 9-feet to 10.4-feet when he used a Werner A-frame ladder to access and egress the second floor. The employer did not train the employee how to recognize and minimize the hazards associated with using ladders on the jobsite.
