29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b)  On or about April 24, 2013:  (a) An employee performing roofing operations to include removal and replacing roofing materials due to storm damage were exposed to fall hazards of approximately 11 feet while working on a roofing system that had a pitch of 12 in 12, without the use of fall protection measures.

29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:  On or about April 24, 2013:  (a) An employee performing roofing operations to include removal and replacing roofing materials due to storm damage were exposed to fall hazards of approximately 14 feet while wearing a harness where the attachment point was not located at the center of the wearers back or above their head.
