29 CFR 1926.501(b)(13):     Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b)    On or about August 22, 2012 employees were working on a roof performing roofing work without fall protection exposing employees to a fall hazard of 9.5 to 19.5 feet.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.1053(b)(1):     Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access.    On or about August 22, 2012 employees using a ladder that did not extend 3 feet above the level the employees were accessing exposing employees to a fall hazard of 9.5 feet.    Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
