29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: a) On August 22, 2012, and times prior thereto, the employer failed to provide strain relief to a flexible extension cord on the floor at the pantry office entrance, with outer insulation pulled back from the male end, employees to electrical hazards not limited to overheating, fire, and/or electric shock.
