29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold shall be protected by a personal fall arrest system: a) On or about August 24, 2012, at a jobsite located at 1520-22 East 69th Street, Chicago, IL - an employee was installing windows from a ladderjack scaffold approximately sixteen (16) feet above the ground without the use of a personal fall arrest system, thereby exposing employees to fall hazards.
