29 CFR 1926.501(b)(13):     Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b):    On the roof of the residence located approximately at 1202 S 6th Street, Norfolk, NE 68701: Employees of Rocabagu Construction Inc. were exposed to fall heights of more than 6 feet while installing an asphalt shingle roof on the main dewelling an detached garage. Employees had personal fall arrest systems available to them however the employer did not require that they be used.

29 CFR 1926.1053(b)(1):     Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    At the residence located at 1202 S 6th Street Norfolk, NE 68701: Employees of Rocabagu Construction were allowed to use half of a 20' fiberglass portable extension ladder to access the roofs of the main dwelling and the detached garage to perform roofing installation work.

29 CFR 1926.1060(a):  The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these:    Rocabagu Construction on the site located at 1202 S 6th Street, Norfolk, NE :     Employees of Rocabagu Construction were not trained by their employer to recognize hazards related to the unsafe uses of ladders. During the inspection the CSHO observed employees utilizing a section of a 20 fiberglass extension ladder to access a roof to work. The ladder did not extend a minimum of 3 past the level being accessed, nor was it secured to prevent displacement.
