29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system:  a) Residential jobsite: On March 20, 2014 and at times prior thereto employees engaged in roofing activities on a 6 in 12 pitched roof without a fall protection system in place.  Employees were exposed to fall hazards ranging from 10 to 20 feet.    Rocabagu Construction, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 594738, citation number 1, item number 1, and was affirmed as a final order on October 31, 2012, with respect to a workplace located at 1202 South 6th Street, Norfolk, NE, 68702.    Note) Abatement certification and documentation ARE required for this item.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access:  a) Residential jobsite: On march 20, 2014 and at times prior thereto employees engaged in roofing activities used an extension ladder for access to their work area.  The ladder extended approximately one foot above the eave of the roof.     Rocabagu Construction, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 594738, citation number 1, item number 2, and was affirmed as a final order on October 31, 2012, with respect to a workplace located at 1202 South 6th Street, Norfolk, NE, 68702.

29 CFR 1926.1060(a)(1)(iii): The employer did not ensure the each employee had been trained by a competent person in the proper construction, use, placement, and care in handling of all stairways and ladders:  a) Residential jobsite: On March 20, 2014 and times prior thereto employees engaged in roofing activities did not acknowledge and/or recognize the hazard of setting up and using an extension ladder that was not extended 3 feet beyond the roofs eave.      b) Residential jobsite: On March 20, 2014 and times prior thereto employees engaged in roofing activities did not acknowledge and/or recognize the hazard of setting up and using an extension ladder on a firm, smooth surface with the ladder shoes in the vertical position.    Note) Abatement certification IS required for this item.
