29 CFR 1926.453(b)(2)(v):  A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:    Roof, Rockland Center - An employee who was not wearing a fall protection harness and lanyard entered the basket of Genie, S-40 aerial work platform from the roof of the school building and was exposed to a potential fall of 24 feet to the ground below on or about August 23, 2012.     Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    Roof, Rockland Center - The employer did not ensure an employee on the roof above the east entrance was protected with conventional fall protection including a guard rail, safety net or personal fall arrest harness attached to an authorized anchor point and thereby exposed to a 13 foot fall from the roof to the concrete sidewalk below on or about August 23, 2012.     Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
