29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8m) or more above lower levels were not protected by guardrail systems, safety net systems, or personal fall arrest systems:    (a)  Meadowland Homes, Inc., 160 Neil Road, Shippensburg, PA - Employees were exposed to an approximate twnety (20) - twenty-five (25) foot fall hazard while laying the sheathing on a steep-sloped (8:12) roof of a new house without a fall protection system, on or about August 23, 2012.    (b)  Meadowland Homes, Inc., 160 Neil Road, Shippensburg, PA - An employee was exposed to an approximate ten (10) foot fall hazard while working off of a roof edge cutting sheets of sheathing which was elevated by a Bobcat Versahandler, on or about August 23, 2012.    No abatement certification or documentation required.

29 CFR 1926.503(a)(1): A training program was not provided for each employee who might be exposed to fall hazards:    (a)  Meadowland Homes, Inc., 160 Neil Road, Shippensburg, PA - Employees were laying sheathing on a steep-sloped (8:12) roof of a new house and did not receive fall protection training, thus exposing the employees to possible fall hazards and related injuries, on or about August 23, 2012.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

Guarding did not meet the requirements as set forth in American National Standards Institute, B15.1-1953 (R1958), Safety Code for Mechanical Power-Transmission Apparatus:    (a)  Meadowland Homes, Inc., 160 Neil Road, Shippensburg, PA - Employees using a Campbell Hausfield vertical air compressor to power their air tools were exposed to a caught in/crushing hazard when the guard was removed, on or about August 23, 2012.    No abatement certification or documentation required.
