29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. Jobsite: The employee, engaged in painting from the elevated platform of the aerial lift, was not trained to recognize the hazards associated with aerial lifts or the procedures to control them.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: Jobsite: 1) The personal fall arrest harness, worn by the employee in the aerial lift, was not properly rigged and exposed him to falls to lower levels. 2) The fall arrest lanyard, worn by the employee working in the basket of the aerial lift, was too long and exposed the employee to contact with lower levels.
