29 CFR 1926.501(b)(1):  Each employee on a walking/working surface having an unprotected side or edge which was six feet (1.8m) or more above a lower level was not protected from falling by the use of a guardrail system, a safety net system, or personal fall arrest system.    (a)  Environmental Demolition, Inc., 6198 Franklin Street, Denver, CO 80216:  On and before August 30, 2012, the employer did not ensure each employee on a walking/working surface having an unprotected side or edge which was six feet (1.8m) or more above a lower level was protected from falling by the use of a guardrail system, a safety net system, or personal fall arrest system in that an employee was working on the pin-house of boiler unit #2 without the use of a fall protection system. The employee was removing asbestos containing material while working on the roof 10 stories from the ground and fell onto a scaffold below.  This condition exposed employees to a fall hazard of approximately 106 feet to the ground below.

29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee who might be exposed to fall hazards:    (a)  Environmental Demolition, Inc., 6198 Franklin Street, Denver, CO 80216:  On and before August 30, 2012,  the employer did not ensure that each employee engaged in roofing operations were trained to recognize the hazards of working at elevation and trained in the use of fall protection. An employee was working on the pin-house of boiler unit #2 without the use of a fall protection system. The employee was removing asbestos containing material while working on the roof 10 stories from the ground and fell onto a scaffold below.  This condition exposed employees to a fall hazard of approximately 106 feet to the ground below.
