The assessment was not completed in time to comply with requirements which are triggered by exposure data or the lack of a "negative exposure assessment," and to provide information necessary to assure that all control systems planned were appropriate for that operation and worked properly:    a)  On or about June 25, 2014, and at times prior, employees were exposed to unknown quantities of airborne asbestos fibers while removing asbestos containing materials including floor tile and mastic because the employer failed to complete an initial exposure assessment in a timely manner.  Personal air samples were collected but had still not been sent for lab analysis about six days after the removal began.
