29 CFR 1926.451(g)(1)(i):     Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold shall be protected by a personal fall arrest system;    1. Jobsite, 236 West Market Street, Building Facade - Employees were conducting siding installation activities from a ladder jack scaffold 11' above ground level without a personal fall arrest system. Observed on or about August 29, 2012.

29 CFR 1926.454(a):     The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training shall include the following areas, as applicable:    1. Jobsite, 236 West Market Street, Building Facade - Employees were conducting siding installation activities from a ladder jack scaffold 11' above ground level without scaffold safety training. Observed on or about August 29, 2012.

Jobsite, 236 West Market Street - The employer did not create and/or maintain a written safety and health program where employees were conducting siding installation activities from a ladder jack scaffold without a personal fall arrest system or protection from electrical hazards. Observed on or about August 29, 2012.
