29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked between the front uprights and the guardrail supports:      a)   Employees were exposed to fall hazard when working/ standing on single plank scaffold platform.  The employer was using a scaffold to install roof shingles; elevation 30 ft.       Location: 519 E 5th Street, Brooklyn, N.Y.      On or about 1/12/2016      NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE CASEFILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION NOR DOCUMENTATION OF ABATEMENT OF THIS ITEM IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activity on low slope roofs, with unprotected sides and edges 6 feet (1.8m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems:      a)  Employee was exposed to 30 ft fall hazard working at the edge of the roof without fall protection. The employer was installing shingles on the penthouse roof.       Location: 519 E 5th Street, Brooklyn, N.Y.
