Worksite, 1001 Crooked Hill Rd. Brentwood NY - An amusement park ride operator was struck by the spinning bucket of a ride called The Scat, while standing below the operators platform; on or about 09/01/12.    Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

29 CFR 1910.147(c)(1):   The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative    a)  Worksite, 1001 Crooked Hill Rd. Brentwood NY - Employees perform maintenance functions on machines such as but not limited to the Scat, Merry-Go-Round, and Ferris Wheel.  There was no Lockout Tagout program; on or about 09/01/12.      Note:  In addition to abatement certification the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.
