29 CFR 1926.1051(a):  Stairways or ladders were not provided  at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided:    a) An employee accessed a platform elevated by a CareLift Zoom Boom telehandler rough terrain forklift by going through a window opening on the third floor. The bottom of the window opening as approximately 37 inches above the floor and a stairway or ladder was not provided. The employee was exposed to a fall to the ground of approximately 26 feet. 29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above the lower levels was not protected by a guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems the employer shall develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502:    a) Employees were not protected from the unprotected side or edge on the walking working surface on the third floor and were exposed to a fall of approximately ten feet to the second floor.    b) Employees were exposed to falls up to approximately17 feet through window openings on the second floor which did not have top rails.
