29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by   employee(s) working from an aerial lift:                                                                                                              This violation occurred on August 30, 2012, at a jobsite located at 8120 West Camp Wisdom Road, Dallas, Texas 75249, were at least three (3) employees operated an aerial lift without wearing body belts with lanyards attached to the boom or basket.

29 CFR 1926.501(b) (1):  Each employee on a walking/working surface with an unprotected side or edge   which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:     This violation occurred on August 30, 2012, where at least two (2) employees, on the northeast side of the jobsite, were working from the unfinished third floor without the use of fall protection and were exposed to fall hazards.
