29 CFR 1926.95(b): Where employees provide their own protective equipment, the employer was not responsible to assure its adequacy, including proper maintenance, and sanitation of such equipment: On or about 08/23/12, employees were exposed to a fall from height hazard of approximately 25 feet to the concrete ground below, while using a Genie, S-60 aerial lift to take measurements of the exterior of the building. The employee was not wearing a lanyard, and the lanyard that he had in his possession was defective in that hook/latch did not spring closed and the lanyard appeared to be stretched.
