The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards: a) Jobsite, 1521 Kenneth Avenue, Arnold, PA: On or about August 29, 2012, prior to roofing activities, the employer did not ensure that fall protection training was provided for employees where exposed to falls up to approximately 20 feet. Employees either used no fall protection while working at heights, or would tie a rope to one worker while the another worker held the opposite end of the rope to prevent falls.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder: a) Jobsite, 1521 Kenneth Avenue, Arnold, PA: On or about August 29, 2012, during roofing activites, the employer did not ensure that the ladder, used to get up/down to/from the porch roof was extended only approximately 18" above the roof line, which exposed an employee to a fall hazard of approximately 15 feet.
