29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems:    (a)  On or about September 5, 2012, KPJ Solution, as controlling contractor, did not protect workers from fall hazards while they were conducting roofing activities on a roof in excess of six feet above the lower surface.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet above the upper landing surface being accessed:    (a)  On or about September 5, 2012, KPJ Solution, as the controlling contractor, did not protect workers from being exposed to fall hazards while using portable ladders that did not extend at least 3 feet above upper landing surface being accessed.
