Throughout the facility, on or about 09/12/12:  The facility exposure control plan was inadequate in that it did not address the requirements listed in paragraphs (d), (e), (g) and (h).    Abatement certification must be submitted for this item.

Within the facility, on or about 11/15/12:  The employer did not ensure that all employees having occupational exposure to blood and/or other potentially infectious materials (OPIM) signed a declination form which contained the verbiage as listed in appendix A for the standard.    Abatement certification must be submitted for this item.

Within the facility, on or about 11/15/12:  The employer did not maintain medical records containing employees hepatitis B vaccination status for all employees having occupational exposure to blood and/or other potentially infectious materials (OPIM).    Abatement certification must be submitted for this item.
