Location: 211 Summer Street, Lynnfield, MA    On or about 9/12/12, the employer did not ensure that workers utilized the extension ladders at a four to one ratio as required, exposing workers to fall hazards of up to 20-feet, 2-inches.

29 CFR 1926.1053(b)(16):     Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired.    Location: 211 Summer Street, Lynnfield, MA    On or about 9/12/12, the employer exposed workers to fall hazards of up to 20-feet, 2-inches when the employer did not remove the following defective extension ladders from service;    1. 40-foot Louisville extension ladder was structurally damaged and bent.  2. Wernher 32-foot extension ladder missing the rope to extend and retract the ladder and the feet of the ladder were loose.
