29 CFR 1910.1030(d)(3)(ii):     The employer did not ensure that the employee used appropriate personal protective equipment when there was occupational exposure:    a) Soiled Utility Rooms Throughout Facility - On or about 10/1/12, employees rinse heavily soiled items in the hoppers.  Employees do not wear personal protective equipment to protect their eyes and mucous membranes from potential splashing, nor were there shields on the hoppers to protect the employees.    ABATEMENT CERTIFICATION REQUIRED

29 CFR 1904.30(a) - A separate OSHA 300 log was not kept for each establishment that was in operation for one year or longer:    a) Office - On or about 9/13/12, the employer did not maintain separate OSHA 300 logs for all three of its establishments (Schofield Residence, Long-Term Home Health Care, and Adult Daycare).  The OSHA 300 logs for 2008 - 2011 had injuries, which occurred at all three locations, recorded on a single establishment record.    NO ABATEMENT CERTIFICATION REQUIRED
