29 CFR 1926.102(a)(1):  Eye and face protective equipment was not used when machines or operations presented potential eye or face injury:    a)  On or about August 13, 2012, roofing employees were exposed to eye injuries while they used pneumatic nail guns without eye protection. 29 CFR 1926.501(b)(13):  Employee(s) engaged in residential construction activities 6 feet (1.8m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were other provisions of paragraph (b) of this section used to provide an alternative fall protection measure:    a)  On or about August 15, 2012, employees were exposed to falls of greater than 6 feet while they conducted re-roofing activities without fall protection measures in place.      Alvarado's Roofing, Inc. was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13) or its equivalent which was contained in OSHA inspection 314660010, Citation 1, Item 1, issued October 22, 2010, with respect to a workplace located at 10711 Idalia Way, Commerce City, CO 80022.  This citation became final on November 19, 2010.    Abatement Note:  Abatement certification and documentation are required for this item (see enclosed Certification of Corrective Action Worksheet).
