29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section:      On September 30, 2012, at an establishment located at 6500 W. Singleton Boulevard, Dallas, Texas; on the southeast quadrant of the facility, at the 96-inch pipe, an employee marking the bottom of an excavation was exposed to a cave-in hazard.
