29 CFR 1926.501(b)(7)(ii): Each employee at the edge of a well, pit, shaft, or  similar excavation 6 feet (1.8 m) or more in depth was not protected from falling by guardrail systems, fences, barricades, or covers:    On the corner of Quaker and the Loop in Lubbock, TX employees engaged in using the electrical outlets were exposed to falls of over 6 feet because no guard rail was put into place at the edge of the pit.

29 CFR 1926.651(l): Guardrails complying with 1926.502(b) were not provided where walkways were 6 feet (1.8m) or more above lower levels:    On the corner of Quaker and the Loop in Lubbock, TX employees engaged in installing a diversion box were exposed to fall hazards because the 2x4 walkways they were using to get out of the trench were lacking any guardrails.

29 CFR 1926.652(b)(1)(i): Excavations were not sloped at an angle not steeper than one and one-half horizontal to one vertical (34 degrees measured from the horizontal) and no other options listed in paragraph (b) of this section were used.    On the corner of Quaker and the Loop employees engaged in trenching activities are exposed to crushing hazards because the slope of the trench is steeper than 34 degrees.
