29 CFR 1904.29(b)(6):  The employer did not enter "privacy case" in the space normally used for the employee's name on the OSHA 300 form or equivalent.     a) Park Pleasant Nursing and Rehabilitation - The employer entered the employee's name and did not enter "privacy case", on the Report of Employee Accident or Near Miss (OSHA 301 Form), for an employee receiving a needlestick.  Observed on or about 12/19/12.      Note:  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.1030(c)(2)(i):  The employer having employees with occupational exposure did not prepare an exposure determination:    a) Park Pleasant Nursing and Rehabilitation - Employees are exposed to bloodborne pathogens during patient care, and the exposure control plan did not include an exposure determination.  Observed on or about 8/16/12.      Note:  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
