29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: (a) On or about 10/3/12 in the Grinding room; a relocateable power tap was being to supply power to the Baldor grinder and sump pump exposing employee to contact with electric current. b) On or about 10/3/12 in the Grinding room; a relocateable power tap was plugged into a cord set which was plugged into a wall outlet exposing employee to contact with electric current. c) On or about 10/3/12 in the Grinding room; a Westinghouse brand motor starter ( #B100-3, Size M-1, Cat# BTOCM1CX ) is being used on an almost daily basis as a switch to turn on and turn off the hollow grinding machine.
