29 CFR 1910.212(a)(1):     Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by contacting the rotating grinding wheel on the edger machines:  a) On or about 10/3/12 in the Grinder area; employees are inserting and removing scissor blades from the Edger machine at a distance of approximately two inches away from an unguarded ( 24" diameter ) grinding wheel exposing them to contact with the rotating wheel operating at a speed of 1745 rpms. 29 CFR 1910.303(b)(2):     Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  (a) On or about 10/3/12 in the Grinding room; a relocateable power tap was being to supply power to the Baldor grinder and sump pump exposing employee to contact with electric current.    b) On or about 10/3/12 in the Grinding room; a relocateable power tap was plugged into a cord set which was plugged into a wall outlet exposing employee to contact with electric current.    c) On or about 10/3/12 in the Grinding room; a Westinghouse brand motor starter ( #B100-3, Size M-1, Cat# BTOCM1CX ) is being used on an almost daily basis as a switch to turn on and turn off the hollow grinding machine.
