Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 1926.502(k) developed and implemented:     (a) Front of house: on 8/16/12 and at times prior thereto, four workers on a low-sloped roof performed roofing work without fall protection. 29 CFR 1926.1053(b)(1):    Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    (a) Front of house: on 8/16/12 and at times prior thereto, an employee descended a ladder while the ladder did not extend 3 feet above the roof edge.
