29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle:    a) On or about October 4, 2012 the interlock doors on the Viper VMC-1270 CNC machine was not installed properly.  Operators were able to run the CNC without the doors being closed.     b) On or about October 4, 2012 the interlock doors on the Viper VMC-2100 CNC machine was not installed properly.
