29 CFR 1926.452(k)(1):  Ladder jack scaffold platforms were not erected so as to limit the height of the platform to 20 feet (6.1 m):    a. On or about October 8, 2012, two employees were working on a ladder jack scaffold platform that exceeded 20 feet in height, in that the platform was elevated 22 feet 4 inches.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used:    a. On or about October 8, 2012, two employees did not have a safe means of access/egress to a ladder jack scaffold system, exposing the employees to a fall hazard.
