OSH ACT of 1970 Section (5)(a)(1):     The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to burns.      a. Z-Cal, Hydro-Therm  On or about October 3, 2012 39 inches of insulation was missing from the lower section a steam pipe for the Hydro-Therm, Z-Cal.

29 CFR 1910.303(b)(7)(i):     Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings shall be effectively closed to afford protection substantially equivalent to the wall of the equipment.    a. #2 Comp Line - On or about October 3, 2012 The following electrical cabinets had openings in their covers:    1. (Un-used opening) the disconnect/reset lever was removed from electrical cabinet #T429 for the 2.5 inch extruder.      2. (Not completely closed) screws were missing from the digital monitor located in the center of electrical cabinet #T408.  Approximately one-half inch gap was between the top of the monitor and cabinet cover.
