29 CFR 1926.20(b)(1):  The employer did not initiate and maintain such programs as may be necessary to comply with this part.    At the job site, 525 N. Broadway, the employer had not developed a program addressing safe work practices when employees work from articulating boom lifts.  Employees were exposed to a fall hazard of approximately 40 feet to the ground.

29 CFR 1926.453(b)(2)(v):   A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:    At the job site, 525 North Broadway, Wichita, Kansas, Two employees were working from an aerial lift "JLG S60SJ" without being tied to the basket with a harness and lanyard.  Employees were exposed to a fall hazard of approximately 40 feet to the pavement.
