29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    a) Jobsite, 840 Saw Mill Run Boulevard, Pittsburgh, PA: On or about October 10, 2012, during roofing activities on a roof with an approximate 6 in 12 pitch, the employer did not ensure that fall protection systems/devices were used where workers were exposed to falls of up to approximately 25 feet.
