29 CFR 1926.300(b)(2): Belts, gears, shafts, pulleys, sprockets, spindles, drums, fly wheels, chains, or other reciprocating, rotating or moving parts of equipment shall be guarded if such parts are exposed to contact by employees or otherwise create a hazard. Guarding shall meet the requirements as set forth in American National Standards Institute, B15.1-1953 (R1958), and Safety Code for Mechanical Power-Transmission Apparatus. On or about October 10, 2012, two employees of Payne Concrete performing grade measurements and assisting the operator/foreman of the Watson 1100 Drill rig were exposed to the rotating shaft and auger bit. The rotating shaft was not guarded or barricaded to protect the employees from caught-in or struck-by hazards.
