29 CFR 1910.147(c)(1):     The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    The employer did not establish and implement a lockout/tagout program consisting of energy control procedures, employee training and periodic inspections for employees exposed to the unintentional startup of the belt sander and polisher/grinder when changing belts and grinding wheels.

29 CFR 1910.134(c)(1):     In any workplace where respirators are necessary to protect the health of the employee or whenever respirators are required by the employer, the employer shall establish and implement a written respiratory protection program with worksite-specific procedures. The program shall be updated as necessary to reflect those changes in workplace conditions that affect respirator use. The employer shall include in the program the following provisions of this section, as applicable:    The employer did not establish and implement a written respiratory program for employees required to wear tight fitting half-mask respirators when polishing which included the following elements:    a) Procedures for selecting respirators;  b) Medical evaluations for employees required to wear respirators:  c) Fit-testing procedures;  d) Procedures and schedules for cleaning, disinfecting, storing, inspecting, repairing, discarding and otherwise maintaining respirators;  e) Training of employees in the respiratory hazards to which they are potentially exposed during routine and emergency situations;  f) Procedures for regularly evaluating the effectiveness of the program.
