29 CFR 1910.147(c)(6)(i)(C):     Where lockout was used for energy control, the periodic inspection did not include a review, between the inspector and each authorized employee, of that employee's responsibilities under the energy control procedure being inspected.     a) Area of 949 Evans Street, equipment storage area - employee(s) are authorized to perform maintenance on a John Deere 410D Loader/Backhoe, where energy included gravity and mechanical energy,  without receiving periodic inspections of that employees responsibilities under the energy control procedure being inspected.  Neither the consultant nor a Plasterer authorized employee conducted a review for each authorized employee, on or about October 12, 2012.

29 CFR 1910.147(c)(7)(i)(A):     Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation.     a) Area of 949 Evans Street, equipment storage area - employees are authorized to perform maintenance to include, but not limited to the servicing of brakes on the John Deere 410D Loader/Backhoe without receiving training in the hazardous energy identification, isolation, and verification methods and techniques necessary for safe maintenance, on or about October 12, 2012.
