29 CFR 1926.403(b)(1):     Examination. The employer shall ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees. Safety of equipment shall be determined on the basis of the following considerations:    (a).  Worksite located at 607 N. Maguire, Warrensburg, Missouri- Employees were exposed to a shock or electrocution hazard in that the extension cord being used by the employees had not been inspected oterwise the employer would have found that the outer insulation was breached and the grounding pin was missing.

29 CFR 1926.404(b)(1)(i):     General. The employer shall use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. These requirements are in addition to any other requirements for equipment grounding conductors.    (a).  Worksite located at 607 N. Maguire, Warrensburg, Missouri- Employees were exposed to a shock or electrocution hazard in that employees were not using a GFCI, nor was the employer using an assured grounding program.

29 CFR 1926.404(f)(6):     Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent and continuous.    (a).  Worksite locateda at 607 N. Maguire, Warrensbug, Missouri- Employees were exposed to a shock or electrocution hazard in that the extension cord as well as the outlet it was plugged in to was not provided with any means of grounding, thus an unknowing could potentially become the path to ground.
