29 CFR 1926.1053(b)(1):  Where portable ladders were used for access to an upper landing surface and the ladder���s length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    (a)  At the project site on or about October 24, 2012, and at times prior thereto, the six foot stepladder used to access the work platform did not extend three feet above the top of the platform.  It was about 5 4 from the top of the work platform to the concrete ground below.
