29 CFR 1926.404(f)(6):     The path to ground from circuits, equipment, or enclosures was not permanent and continuous:      a) On or about 10/25/12, on the south side of the flat roof at the Target Store, 5622 Amanda Ln., Orchard Park, NY: An employee used a Bosch Hammerdrill to attach a piece of metal on a corner. The Hammerdrill was connected to 120 volt-ac power via an extension cord which was plugged into a receptacle on a Honda portable generator. The plug on the extension cord lacked a ground prong.      NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.501(b)(3):     Each employee in a hoist area was not protected from falling 6 feet (1.8m) or more to lower levels by guardrail systems or personal fall arrest systems:    a) On or about 10/25/12, on the north side of the flat roof at the Target Store, 5622 Amanda Ln., Orchard Park, NY: An employee was exposed to a potential fall of approx. 20 ft. from the high roof to the ground while using a mechanical hoist to lift ladders and containers of roofing adhesive from the ground to the roof. A warning line had been set up in the area, but no guardrail system or personal fall arrest system was utilized.    NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.501(b)(10):     Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.          a) On or about 10/25/12, on the south side of the flat roof at the Target Store, 5622 Amanda Ln., Orchard Park, NY:  An employee was exposed to a potential fall of approx. 11 ft. from the high roof to a lower roof while standing on top of, and walking along a parapet wall, pushing a Thermal Welding Machine ahead of him to weld the seams of the EPDM Rubber Roofing System together. No fall protection system was utilized.    NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.416(e)(1):     Worn or frayed electric cords or cables were used:    a) On or about 10/25/12, on the south side of the flat roof at the Target Store, 5622 Amanda Ln., Orchard Park, NY: An employee used a DeWalt Reciprocating Saw to cut roofing material. The cord on the saw was frayed at the handle exposing the conductors.    NO ABATEMENT CERTIFICATION REQUIRED
