29 CFR  1926.501(b)(10):Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  On or about October 8, 2020, at a worksite located at 26151 Lakeshore Blvd, Euclid, OH 44132 during roofing work on a low slope roof greater than 50 feet in width, the employer did not ensure employees were protected from fall hazards of approximately 20.7 feet by a fall protection system/device.  Preferred Roofing Services LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(10), which was contained in OSHA inspection number 1106603, citation number 02  item number 001 and was affirmed as a final order on December 30, 2016 , with respect to a workplace located at 4240 Sunnybrook Rd. Kent, OH 44240.
